Insurance
Every effort is being made to keep down the cost of dental
care. We are happy to cooperate with you and your insurance
company. In order to help you receive the maximum benefit your
insurance provides, we ask that you provide our front staff with
required information such as insurance mailing address, group
number, identification numbers and phone numbers. This way we
may verify and help you understand your insurance benefits. Your
insurance policy is a contract between you and your insurance
company, please remember that you are ultimately responsible for
payment of fees to the office. Any balance remaining after 60
days is your responsibility and must be paid in full by you.
Financial Policy
Payment is due at the time services are rendered. We offer an
accounting courtesy for procedures $500.00 and over when paid in
full at the start of treatment, because we do not have the
expense of running statements, envelopes, postage and carrying a
balance we gladly pass the savings on to you. Cash and checks
are given a 5% courtesy, Credit cards are given a 3% courtesy.
Financing is available through outside agencies on approved
credit.
Patient Forms
New patients are required to complete our Patient
Registration and Consent Forms. For your convenience you can download
both of the form and complete it in advance of your
appointment. If you have any questions or would like us to mail
you a form, please contact us.
